OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards from natural gas: a) At the fire pit on the patio of restaurant - On or around the dates of June 5 through June 9 of 2018, while turning the natural gas fire pit on and off, employees were exposed to fire and explosion hazards due to a malfunctioning igniter and not having the correct tool to operate the gas valve. The employer did not provide the correct tool for employees to operate the gas valve. Rather than using the manufacturer provided valve key, employees used pliers. On June 9, 2018, the valve broke while turning off the gas valve with pliers resulting in a gas explosion causing two employees to be injured.
