29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:                  a) On or about June 12, 2018, employees were exposed to eye injury while operating pneumatic nail guns and electric saws without the benefit of eye protection.                 In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):          a) On or about June 12, 2018, employees were exposed to the hazard of falling while working without fall protection from a 10/12 pitched roof surface having a ground to eave height of approximately 12 feet.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:              a) On or about June 12, 2018, employees were exposed to the hazard of falling from elevations up to approximately 12 feet when required to climb portable ladders while carrying materials to a roof deck landing surface.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
