29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) Job site, 2237 Saw Mill Run Boulevard, Pittsburgh, PA: On or about June 7, 2018, during roofing activities on the roof above the front show room, the employer did not ensure that appropriate fall protection systems and devices were in use where fall hazards of up to approximately 14 1/2 feet existed in that the parapet wall on this level was determined to be only approximately 20 inches tall and no other form of fall protection was in use. b) Job site, 2237 Saw Mill Run Boulevard, Pittsburgh, PA: On or about June 7, 2018, during roofing activities on the lower roof between the front show room and the garage, the employer did not ensure that appropriate fall protection systems and devices were in use where fall hazards of up to approximately 11 1/2 feet existed in that parapet wall on his level was determined to be only approximately 27 inches tall and no other form of fall protection was in use.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder and ladders were not set up such that ladder deflection under a load would, by itself, cause the ladder to slip off its support: a) Job site, 2237 Saw Mill Run Boulevard, Pittsburgh, PA: On or about June 7, 2018, during roofing activities on the lower roof between the front show room and the garage, the employer did not ensure that the extension ladder was not used safely, as it was set up adjacent to the roof, not permitting employees to use both hands when getting on/off the ladder and allowing it to become displaced when lateral force was exerted on it as employees got on/off the ladder.
