29 CFR 1910.178(k)(1): The brakes of highway truck(s) were not set and wheel chocks placed under the rear wheels to prevent truck(s) from rolling:  Dock Door 23: On May 22, 2018, tractor/trailer brakes were not engaged and wheel chocks were not used while a powered industrial vehicle loaded the trailer. Employees were exposed to struck-by, fall, and caught-between hazards when the tractor/trailer rolled away from the dock and the powered industrial vehicle fell to the lower level.  The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.178(k)(1)], which was contained in OSHA inspection number 1114625, citation number 1, item number 1 and was affirmed as a final order on June 12, 2017, with respect to a workplace located at 1025 W. 20th Street, Oshkosh, Wisconsin 54902.
