29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. a) Employees were exposed to 120/208V energized parts and arc flash hazards when working on electric equipment that had not been de-energized; electric panel MP 14 had a arc-flash explosion. --- Employees did not wear arc flash PPE (personal protective equipment); arc flash suit, hood, face shield, gloves, rubber mats, barricades; Location: 200 E 59th Street, New York, N.Y. On or about 6/9/2018 Top Shelf Electric Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 1926.28(a), which was contained in OSHA inspection number1141807, citation number 01, item number 01 and was affirmed as a final order on August 17, 2017, with respect to a workplace located at 203 East 92nd Street, New York, NY.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. a) Employees were exposed to 120/208 v 800 amp electric shock hazard when working on electric equipment that had not been de-energized nor were the employees protected by other means; electric panel MP 14 had a short-circuit arc-flash explosion. --- Employer did not de-energize electric panels MP 14 and MP 24 before having employees work on the panels. Location: 200 E 59th Street, New York, N.Y. On or about 6/9/2018 Top Shelf Electric Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.416(a)(1), which was contained in OSHA inspection number 1141807, citation number 01, item number 02(a) and was affirmed as a final order on August 17, 2017, with respect to a workplace located at203 East 92nd Street, New York, NY.
