29 CFR 1910.217(c)(2)(iv): The interlocked press barrier guard(s) on mechanical power press(es) were not attached to the press frame or bolster:    The employer did not ensure that the interlocked barrier guard was attached to the press frame, on a full revolution continuous operation power press, identified as priming unit #5, located in the Primer Manufacturing Area.  Employees were exposed to machine hazards associated with moving parts and nip points when accessing the point of operation from the back of the machine.

Employees were exposed to machine hazards associated with nip points when accessing the point of operation from the back of the machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a)     On or about May 31, 2018, employees were exposed to machine hazards associated with moving parts and nip points during servicing and cleaning of the punching mechanism, on full revolution continuous operation power press, identified as priming unit #5, located in the Primer Manufacturing Area. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.    Olin Corporation, Winchester Division was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(d), which was contained in osha inspection number 1051184, citation number 1, item number 2 and was affirmed as a final order on 05/31/2016, with respect to a workplace located at 600 Powder Mill Road, East Alton, IL 62024.
