29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    On or about June 14, 2018, the employer did not ensure that each affected employee used appropriate eye protection while using woodworking equipment, such as the Dewalt compound miter saw. Employees were exposed to eye injuries from flying wood chips.

29 CFR 1910.213(c)(1): Circular hand-fed ripsaw(s) were not guarded by an automatically adjusting hood, which completely enclosed that portion of the saw above the table and above the material being cut  On or about June 14, 2018, inside both shops at 999 Tidwell Rd. West Monroe, LA 71292, the employer allowed employees to operate a Powermatic table saw Model  66-TA S#03030660707 which was not guarded at the blade, exposing employees to caught-in hazards.

29 CFR 1910.213(c)(2): Each hand-fed circular ripsaw shall be furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. The spreader shall be made of hard tempered steel, or its equivalent, and shall be thinner than the saw kerf. It shall be of sufficient width to provide adequate stiffness or rigidity to resist any reasonable side thrust or blow tending to bend or throw it out of position. The spreader shall be attached so that it will remain in true alignment with the saw even when either the saw or table is tilted. The provision of a spreader in connection with grooving, dadoing, or rabbeting is not required. On the completion of such operations, the spreader shall be immediately replaced.  On or about June 14, 2018, inside both shops at 999 Tidwell Rd. West Monroe, LA 71292, the employer allowed employees to operate a Powermatic table saw Model  66-TA S#03030660707 which was not installed spreader devices, exposing employees to struck by hazards.

29 CFR 1910.213(c)(3): Each hand-fed circular ripsaw shall be provided with nonkickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut.  On or about June 14, 2018, inside both shops at 999 Tidwell Rd. West Monroe, LA 71292, the employer allowed employees to operate a Powermatic table saw Model  66-TA S#03030660707 which was not guarded with an anti-kickback device , exposing employees to struck by hazards.
