29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    On or about June 12, 2018 at a worksite located at 1464 Town Center Blvd., during commercial masonry activities, employees were exposed to fall hazards of approximately 16 feet. The employer failed to ensure fall protection systems/devices were used by all employees working from a JLG 800S aerial lift.
