29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit.    a) Site - On or about 06/11/18 and at times prior thereto employees  engaged in roofing work came in close proximity of the uninsulated crimped splice terminals connecting the 120V/240V service drop from the pole transformer to the house meter, and the employer did not assess the electrical hazards prior to the commencement of the work.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.416(a)(1): The employer permitted employees to work in proximity of electric power circuits that the employees could contact during the course of work, and the employees were not protected against electric shock by de-energizing the circuit and grounding it, or by guarding it effectively by insulation or other means.  a) Site - On or about 06/11/18 and at times prior thereto employees  engaged in roofing work came in close proximity of the uninsulated crimped splice terminals connecting the 120V/240V service drop from the pole transformer to the house meter, and the employer did not protect employees from accidental contact with energized parts.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails    a) Site - On or about 06/11/18 and at times prior thereto employees  engaged in roofing work came in close proximity of the uninsulated crimped splice terminals connecting the 120V/240V service drop from the pole transformer to the house meter. An extensible aluminum ladder was used to reach the roof eave.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a) Site - On or about 06/09/18 and at times prior thereto employees  engaged in roofing work utilized ladders to access sloped roofs and came in close proximity of uninsulated sections of electrical lines. The employer did not develop, implement, and enforce a training program that would enable employees to recognize and eliminate hazards associated with the use of ladders.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
