29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about May 15, 2018, MJM Contracting, Inc., as the controlling employer, did not initiate and maintain a program which provided for frequent and regular inspections of the job site, materials and equipment by a competent person. MAV Enterprises, Inc. dba Paradise Concrete and Landscaping, as the creating and correcting employer, cut a portion of the edge of a walkway floor on the second level of the building that was not protected from fall hazards. Employees of Pachi's Projects LLC, a subcontractor, were exposed to the fall hazards while engaged in drywall operations near the edge of a floor hole on the second level of the building. MJM Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1288675, citation number 1, item number 01 and was affirmed as a final order on March 2, 2018, with respect to a workplace located at 2850 S. Circle Dr., Colorado Springs CO 80906.
