(a) On the roof - An employee working on a suspended scaffold was tied off to a lifeline that was not provided with abrasion protection from sharp edges and was not fastened to a safe point independent of the scaffold; on or about 06/14/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $ 1,000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) On the roof - An employee working on a suspended scaffold was attached to a lifeline that was not fastened independent of the scaffold, the line did not have abrasion protection and the line was not fastened to a hard point rated at 5000 pounds. Another employee tender was working at an unprotected edge without fall protection; on or about 06/14/18.
