29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 13, 2018 at the above addressed jobsite, employees conducting roofing operations, at approximately 9.6-14.7 feet above ground and were not utilizing any fall protection therefore were exposed to a fall.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   On or about June 13, 2018 at the above addressed jobsite, the portable ladder used to access the roof of the garage did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to fall hazards of approximately 9.6 feet.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
