29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage and sharp corners and projections were not avoided:     a) Job site, 8651 Carey Lane, Pittsburgh, PA: On or about May 15, 2018, during construction activities, the employer did not ensure that cords were used appropriately in that they were strewn on the floor in the walking/working areas where they were subjected to being stepped on, and they were exposed to sharp edges on metal stud framing and/or framing components.

29 CFR 1926.453(b)(2)(v): A body belt shall was not work and a lanyard attached to the boom or basket when working from an aerial lift:   a) Job site, 8651 Carey Lane, Pittsburgh, PA: On or about May 15, 2018, during construction activities, the employer did not ensure that an employee who was operating an aerial lift was using appropriate fall protection systems/devices when booming up to the second story of the building.
