This condition exposed employees to cave-in and struck-by hazards while working in a trench excavation approximately 12 feet in depth.     (b)T&R Contracting, Inc. at Intersection of Arrowhead Parkway and South Foss Avenue, Sioux Falls, SD 57110: On or about and at times prior to June 14, 2018, the controlling and exposing employer did not ensure employees were protected from cave-in and struck-by hazards in that, employees accessed a trench excavation by walking between the exterior wall of a trench shield and the vertical excavated wall of the trench at a depth greater than 5 feet. This condition exposed employees to cave-in and struck-by hazards while working in an excavation approximately 12 feet in depth.
