In Excavation #1, where employees were exposed to a crushed-by hazard when installing 6-inch sewer lines at a depth exceeding 4 feet without a safe means of egress. b. In Excavation #2, where employees were exposed to a crushed-by hazard when installing 6-inch sewer lines at a depth exceeding 4 feet without a safe means of egress. c. In Excavation #3, where employees were exposed to a crushed-by hazard when installing 6-inch sewer lines at a depth exceeding 4 feet without a safe means of egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about June 12, 2018, in a new addition of the Wedgewood Falls Subdivision at or near Harmon Drive: a. In Excavation #1, where employees were exposed to a crush-by hazard when installing sewer lines in a trench excavation without the spoil pile placed at least 2 feet from the edge. b. In Excavation #2, where employees were exposed to a crush-by hazard when installing sewer lines in a trench excavation without the spoil pile placed at least 2 feet from the edge.
