29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On or about June 12, 2018, in a new addition of the Wedgewood Falls Subdivision at or near Harmon Drive in Conroe, TX:    a. In Excavation #1, where employees were exposed to a crushed-by hazard when installing 6-inch sewer lines at a depth exceeding 4 feet without a safe means of egress.     b. In Excavation #2, where employees were exposed to a crushed-by hazard when installing 6-inch sewer lines at a depth exceeding 4 feet without a safe means of egress.     c. In Excavation #3, where employees were exposed to a crushed-by hazard when installing 6-inch sewer lines at a depth exceeding 4 feet without a safe means of egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.     On or about June 12, 2018, in a new addition of the Wedgewood Falls Subdivision at or near Harmon Drive:    a. In Excavation #1, where employees were exposed to a crush-by hazard when installing sewer lines in a trench excavation without the spoil pile placed at least 2 feet from the edge.     b. In Excavation #2, where employees were exposed to a crush-by hazard when installing sewer lines in a trench excavation without the spoil pile placed at least 2 feet from the edge.     PRINCIPAL SERVICES LTD., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.651(j)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1121066, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 24, 2016, WITH A FINAL ABATEMENT DATE OF MARCH 16, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT FALCON LANDING BLVD AT GASTON ROAD, KATY, TX 77449.
