29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.     a) Jobsite - The employer did not provide a safe means of access for all levels of the tubular supported welded frame scaffold by the use of portable ladders, hook-on ladders or other acceptable means.  Employees installing windows from the scaffold used the rungs on the scaffold end frames for access.    Noted on or about 6/12/18.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a) Jobsite: 3rd floor, front of building, center, right and left wall openings - Employees preparing to install new windows on unprotected wall openings, where the center opening measured 62 inches in width, 64 inches in height, with an inside bottom edge measuring 28 inches above the walking/working surface, and an outside bottom edge measuring approximately 25 feet above ground level, were exposed to fall hazards without the use of fall protection system.    Observed on or about 6/12/18.
