The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about June 14, 2018: a) Employees removing metal roofing materials were exposed to injuries when the employer failed to ensure that the employees were properly tied off, with the attachment point being in the center of the wearers back.
