On or about June 1, 2018, an employee was working/walking on a scaffold that was not constructed and designed by a qualified person. Scaffold was constructed out of wooden pallets and wooden planks.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.      Location - 7 Progress Avenue, Chelmsford, MA.    On or about June 1, 2018, employee was not provided a safe means of access to the scaffold he was working/walking on. Employee was using a tubular welded scaffold section as a ladder.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers.    Location - 7 Progress Avenue, Chelmsford, MA.    On or about June 1, 2018, employees were not protected from falling while working/walking in close proximity of an open and unprotected manhole. An employee fell into the manhole and suffered injuries.
