29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about June 14, 2018, at or near 2222 Rayford Road, Spring, TX, where employees were exposed to a fall hazard when installing 24-inch storm drain system at a depth exceeding 4 feet without a safe means of egress. HASSELL CONSTRUCTION COMPANY, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1031127, CITATION NUMBER 1, ITEM NUMBERS 5, AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 22, 2017, WITH A FINAL ABATEMENT DATE OF DECEMBER 19, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 900 GOLFVIEW DRIVE, RICHMOND, TX 77469.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about June 14, 2018, and times prior thereto, at or near 2222 Rayford Road, Spring, TX where employees were exposed to a crushed-by hazard when installing 24-inch concrete pipe at a depth exceeding 5 feet without a protective system. HASSELL CONSTRUCTION COMPANY, INC.
