29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    (a)  On or about June 14, 2018, and at times prior, workers were exposed to fall hazards of 15 feet and greater while roofing a metal building, without any means of fall protection. 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) On or about June 14, 2018, employees were exposed to fall hazards from the ladder used to access the roof of the metal building under construction, which did not extend above the landing surface, was not secured, and for which grasping devices were not provided.
