29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       On or about June 15, 2018 and at times prior thereto, the employer did not ensure employees were in use of fall protection while working from roof installing asphalt shingles, exposing the employees to a fall hazard of more than six (6) feet to the ground below.            The Rosales Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1064352, citation number 001, item number 02. and was affirmed as a final order on July 21, 2015, with respect to a workplace located at 13104 N. McArthur Blvd Oklahoma City, OK 73142.      The Rosales Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1041028, citation number 001, item number 01. and was affirmed as a final order on July 21, 2015, with respect to a workplace located at 10908 Milford St Oklahoma City, OK 73162.      The Rosales Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1177444, citation number 001, item number 01. and was affirmed as a final order on April 14, 2017, with respect to a workplace located at NW 179th and Groveton Rd Oklahoma City, OK 73115.    The Rosales Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1205442, citation number 001, item number 01 and was affirmed as a final order on May 8, 2017, with respect to a workplace located at 18629 Melgold Edmond, OK 73012.
