29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color: A) United Parcel Service, Inc., located at 801 Industry Avenue, Lima, OH 45804: On or about April 11, 2018, the employer did not ensure that exit signs, located on the West side and the East side of the Package Car Position (PCP) trailer, were illuminated. Employees were exposed to fire hazards in that it would be more difficult to egress the PCP trailer in a case of a fire and limited visibility inside the PCP trailer.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening when they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: A) United Parcel Service, Inc., located at 801 Industry Avenue, Lima, OH 45804: On or about April 11, 2018, the employer did not ensure a clearance of at least three feet in front of the electrical panel located in the Package Car Position (PCP) trailer. The electrical panel powered lights and heaters inside the PCP trailer, and was obstructed by a conveyor in front of it. Employees were exposed to electrical and fire hazards in that it would be more difficult to shut off electrical power to the lights or the heaters inside the PCP trailer in a case of a fire.
