29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):  a) On or about 6/14/18 at a roof replacement project located at 2521 Route 77, Strykersville, New York; employees working from an aluminum ladder jack scaffold stage placed the stage approximately 1 foot away from the electric service cable. Minimum clearance for 300 volts or less is 3 feet.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 6/14/18 at a roof replacement project located at 2521 Route 77, Strykersville, New York; employees were replacing shingles on the roof of a two story house. The roof had a 6 in 12 pitch and an eave height of approximately 18 feet. There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED  WNY First Choice Construction Co Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1290385, citation number 1, item number 2, and was affirmed as a final order on 2/22/18, with respect to a workplace located at 374 Prospect Avenue, Buffalo, New York.  WNY First Choice Construction Co Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1290975, citation number 1, item number 1, and was affirmed as a final order on 2/27/18, with respect to a workplace located at 164 Dewey Avenue, Buffalo, New York.
