29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) Jobsite: 4500 Centre Avenue, Pittsburgh, PA- On or about June 14, 2018, employees were working from stair stringers that were not structurally stable at the time. A tack weld failed and a "c-channel" rolled causing the stair stringer to fall while an employee was working from the stringer; the employee fell approximately 6-7 feet to another stringer below.
