29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage. On or about June 12, 2018: a) A the job site located at 2011 Yuette Way Braselton, Georgia employees were exposed to electrical hazards when a flexible cord used to power an Jenny air compressor on the worksite was not inspected before use by the employer, and had a missing grounding pin.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 12, 2018: a) At the jobsite located at 2011 Yuette Way Braselton, Georgia, employees were exposed to fall hazards of approximately 12.1 feet while installing framing material on a new residential structure without the use of fall protection measures. Rafel Olvvra was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1245288, citation number 1, item number 1 and was affirmed as a final order on August 31, 2017, with respect to a workplace located at 4648 Bogan Meadows Drive in Buford Georgia.
