29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. Van Wert Pallets, LLC - Van Wert, Ohio: On or about June 14, 2018, the employer failed to develop and implement an energy control program that includes training, written procedures, and audits to protect employees who perform maintenance and servicing such as, but not limited to, changing the blade on the chop saw.  Employees were exposed to the unexpected startup of the rotating blade in that the machine was not locked out.

Van Wert Pallets, LLC - Van Wert, Ohio: On or about June 14, 2018, the employer did not ensure that the Clark forklift (s/n GPX230-0272-7965KF) was removed from service during a time in which it was not in safe operating condition.  Employees were exposed to struck-by injuries in that the horn was not operative.

Van Wert Pallets, LLC - Van Wert, Ohio: On or about June 14, 2018, employees that operated the pallet Trim Saw were not protected from amputation injury.  The employer did not ensure the machine's point of operation was guarded.      b. Van Wert Pallets, LLC - Van Wert, Ohio: On or about June 13, 2018, the employer did not ensure the horizontal band saw, used to dismantle pallets, was guarded to protect employees from amputation injuries.
