29 CFR 1910.147(f)(1)(v): Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance. a. At the work site, employees de-energized the #10 Extruder "B" line according to the lock out/tag out procedure prior to adjusting the water flow, then energized the #10 "B" line to verify the adjustment. An employee suffered an amputation when the machine cycled during reattachment of a guard while the machine was still energized.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. At the work site, an employee suffered an amputation when the machine cycled during reattachment of a guard while the machine was still energized. The employer did not ensure that the #10 Extruder line's point of operation was adequately guarded.
