Instance #2 - Caterpillar # 069 - Faulty operator brakes in that brakes allowed coasting when applied.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On June 12, 2018, at the facility located at 1120 N Farm Rd 123,Suite E, Springfield, MO., an employee was exposed caught-in injuries from the rotating parts hazard of un-protected Chain and Sprocket on the Paint Line. As a result, the employee received a partial amputation to his right index finger during a servicing task on the adjoining machine.
