29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition:  a.  Ellsworth Subdivision, 808 Langley Path,  Lot #24, John Creek, GA (Jobsite) - Employees was exposed to struck-by hazard.  The employee was not utilizing face and/or eye protection while chipping the bricks to various sizes using a trowel, to prevent debris, brick, and mortar particles from flying back into the employee's face, on or about 06/19/18.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a. Ellsworth Subdivision, 808 Langley Path,  Lot #24, John Creek, GA (Jobsite) - Employees were exposed to fall hazard.  The employees were not utilizing fall protection while working and walking approximately 12 feet and greater above ground level on the elevated porch roof, installing bricks to the front exterior of the newly constructed residential home, on or about 06/19/18.
