29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Roll forming Department - On or about June 11, 2018, employees were exposed to machine hazards associated with moving parts when greasing rollers on Roll Former #101, which was attached to the Rousselle press. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Roll Forming Department - On or about June 11, 2018, the employer failed to ensure that each of the two employees involved in the servicing task of greasing roller bearings on Roll Former #101 affixed a personal lock onto a group lockout device. Neither employee used a lockout device to lockout out Roll Former #101, thereby the two employees were exposed to hazards associated with the unintentional startup of the equipment.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: a) Roll Forming Department - On or about June 11, 2018, the employer failed to provide adequate training for employees performing service and/or maintenance activities to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired. A maintenance employee sustained fractures to the thumb, and index, middle and ring fingers on the left hand as a result of Roll Former #101 not being de-engergized and locked out during servicing (greasing rollers) of this equipment. Edsal Manufacturing Company, Inc. was previously cited for violation of the Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 996548, citation number 1, item 3 and affirmed as a final order on February 18, 2016, with respect to a workplace location at 4400 S. Packers Ave., Chicago, Illinois 60609.
