29 CFR 1926.300(b)(2):  Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus.  (a) Employee(s) used a RGC Ladder Hoist to convey materials up to the roof. The hoist did not have the power transmission pulley and belt guarded, exposing employees to a caught-in hazard and amputation injury.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  (a) An extension ladder used to access the north side roof of a residential home, did not extend above the roof eave more than three (3) feet and was not secured at the top. This exposed employees to a fall hazard of 20 feet.

This exposed employees to a fall hazard of approximately 20 feet.  The Quality Pros Exteriors LLC company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1298657, citation number 1, item number 1, and was affirmed as a final order on August 10, 2018, with respect to a workplace located at 1728 Moccasin Trail, Waukesha, WI 53186.
