29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by paragraph (l): At the location of 151 S. Cedar Niles Rd, in Gardner KS, an employee engaged in moving a pallet of bricks to a scaffold with an all-terrain forklift (Forklift Model G12-55A SN#0160049605) and workers on and near the scaffold were exposed to struck by hazards in that the employee operating the forklift had not completed forklift training.
