29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Tiffany Square subdivision, Excavation located between lots #18 through #21: On or about 06/19/2018, and at times prior, the employer exposed employees to struck-by head injuries, in that protective helmets were not provided to employees working inside an excavation with large spoil piles on the excavated wall's edge installing 8-inch sewer lines.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  (a) Tiffany Square subdivision, Excavation located between lots 19 to 21: On or about 06/19/2018, and at times prior, the employer exposed employees to engulfment hazards while working in a excavation which measured 13-feet deep x 14-feet wide by 50-feet long, in that a safe means of egress was not provided as employees were engaged in sewer line installation work.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  (a) Tiffany Square subdivision, Excavation located between lot 19 and lot 21: On or about 06/19/2018, and at times prior, the employer exposed employees to engulfment hazards while working in an excavation which was 13-feet deep by 14-feet wide by 50-feet long, in that the spoil pile was not placed at least 2-feet from the excavated wall and no cave-in protection system was used as employees were engaged in sewer line installation.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  (a) Tiffany Square subdivision, Excavation located between Lots #19 and #21: On or about 06/19/2018, and at times prior, the employer failed to inspect the excavation prior to allowing employees to enter, in that employees were exposed to 13-foot deep trench walls plus the height of the spoil pile of 6-feet with no safe means of egress, no cave-in protection system and not wearing the personal protective equipment needed for overhead struck-by hazards.
