29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  (a)  Rip Saw Operation - The employer did not develop or implement an effective hearing conservation program for the employees working on the rip saw who were exposed to an Eight Hour Time Weighted Average (TWA8) of 90.8 decibels in the A Weighted Scale for the 486 minutes sampled on or about June 27, 2018.  (b) Tread Molding Operation - The employer did not develop or implement an effective hearing conservation program for the employees working on the tread molding machine who were exposed to an Eight Hour Time Weighted Average (TWA8) of 89.3 decibels in the A Weighted Scale for the 480 minutes sampled on or about June 27, 2018.  (c) Gluing Operation - The employer did not develop or implement an effective hearing conservation program for the employees working on the gluing machines who were exposed to an Eight Hour Time Weighted Average (TWA8) of 88.8 decibels in the A Weighted Scale for the 489 minutes sampled on or about June 27, 2018.  (d) Molding Operation - The employer did not develop or implement an effective hearing conservation program for the employees working on the molding machine who were exposed to an Eight Hour Time Weighted Average (TWA8) of 88.6 decibels in the A Weighted Scale for the 484 minutes sampled on June 27, 2018.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) Ridgeview Manufacturing, LLC - The employer did not develop or implement a written respiratory protection program for the employees who were required to wear 3M FF 400 negative pressure half-mask respirators during the cleaning out of the dust collection system, on or about June 19, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Ridgeview Manufacturing, LLC - Plantwide - The employer did not develop or implement a written hazard communication program for the employees exposed to skin irritation hazards while using the WD-40 during the maintenance of the machines, on or about June 19, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
