29 CFR 1926.602(b)(3): Safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No. 1 and No. 2 of 1968, and No. 3 of 1969 were not complied with: JOBSITE: The outrigger on the driver's side of the Cormach 30000 WB65-721 Articulating knuckle boom crane (Crane) was not fully extended while the Crane was carrying materials from the upper levels of the building under renovation onto the flatbed of a truck on the ground. On or about 06/14/2018, the Crane tipped over to the driver's side while the boom was carrying 26 pieces of lead lined sheet rocks from the 3rd level of the building, and the tip of the boom with the load fell onto a powered industrial forklift truck (PIT). The operator of the PIT sustained minor injuries from the incident.
