Ellwood Quality Steels Company, East Bay- On or about June 19, 2018, and on dates prior thereto: employer had not evaluated the workplace to determine if 30 inch vertical molds were permit-required confined spaces. Employees periodically entered 30 inch vertical molds during mold set-up operations. The molds contained, or potentially contained, hazards including but not limited to, a hazardous atmosphere and an internal configuration such that an entrant could be trapped by a floor which sloped downward and tapered to a smaller cross-section. The employer did not identify 30 inch vertical molds as permit required confined spaces.

Ellwood Quality Steels Company , East Bay - On or about June 19, 2018 and on dates prior thereto: employees periodically entered 30 inch vertical molds during mold set-up operations. The molds contained, or potentially contained, hazards including, but not limited to, a hazardous atmosphere, and an internal configuration such that an entrant could be trapped by a floor which sloped downward and tapered to a smaller cross-section.
