29 CFR 1926.1203(h)(1)(ii): Before entry operations begin, the host employer did not provide the hazards or potential hazards in each space, or the reason it is a permit space. On or before April 24, 2018, the employer did not provide the subcontractor employer with the hazards or potential hazards of each space for which subcontractor employees were allowed to perform weatherization work activities. This exposed the subcontractor employees to hazardous conditions such as, but not limited to, fall protection and electric shock.
