29 CFR 1926.62(d)(1)(i): The employer did not initially determine if any employee may be exposed to lead at or above the action level: 611 NORWICH AVENUE, TAFTVILLE, CT: On or about June 21, 2018, employees were performing lead abatement activities in the Building 2 stairwell at the above-referenced location. Such activities involved dry scraping of lead based paint that was peeling from the walls in the stairwell. No initial exposure assessment was conducted to determine whether the employees were being exposed to lead at concentrations above the action level of 30 micrograms per cubic meter (ug/m3).

29 CFR 1926.62(d)(2)(v)(B): Until the employer performed an employee exposure assessment as required under 29 CFR 1926.62(d) and determined actual employee exposure, the employer did not provide employees performing the tasks described in 29 CFR 1926.62(d)(2)(i), (d)(2)(ii), (d)(2)(iii), and (d)(2)(iv) with appropriate personal protective clothing and equipment in accordance with 29 CFR 1926.62(g): 611 NORWICH AVENUE, TAFTVILLE, CT: On or about June 21, 2018, employees were dry scraping lead-based paint from the walls of a stairwell located in Building 2 of the above-referenced location. The employees were wearing Tyvek suits that did not cover the employees boots or heads.

29 CFR 1926.62(j)(2)(i)(A): The employer did not make available biological monitoring in the form of blood sampling and analysis for lead and zinc protoporphyrin (ZPP) levels to each employee covered under 29 CFR 1926.62(j)(1)(ii), at least every 2 months for the first 6 months and every 6 months thereafter: COMPANY-WIDE: On or about September 19, 2018, employees who were assigned to tasks involving lead abatement were exposed to lead at or above the action level and were included in the company's medical surveillance program. Monitoring of the employees' blood lead and ZPP levels was not conducted every six months or at intervals recommended by the licensed medical professionals.
