Condition observed on or about 6/20/2018. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

A) Room 44 West: Circuits energizing the PTac A/C and heating unit were not locked out or tagged out for employee(s) who service it within the resident rooms resulting in an employee hospitalization from electrical shock.
