On or about June 6, 2018 at the facility located at 4712 Flat Creek Road in Oakwood Georgia: The employer failed to report an in patient hospitalization to the local OSHA area office within the allowed 24 hours. The in hospitalization of an employee occurred on June 6, 2018, the employer notified the local OSHA office on June 12, 2018.
