29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about June 20, 2018, employees performing roofing activities were exposed to fall hazards of approximately 15.5 feet and the employer did not ensure fall protection was used.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)     On or about June 20, 2018, employees performing roofing activities were exposed to fall hazards of approximately 15.5 feet while working and fall hazards of approximately 11.5 feet while using a portable ladder whose side rails did not extend three feet above the upper landing surface.  The employer did not ensure a competent person inspected the job site for unsafe activity and take the necessary steps to eliminate or control the fall hazards.    Noe Cruz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1269403, citation number 2, item number 1b and was affirmed as a final order on December 11, 2017, with respect to a workplace located at 7724 N Knoxville Ave, Mt. Hawley Country Club, Peoria, IL 61614.    Noe Cruz Martin was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1229749, citation number 1, item number 1b and was affirmed as a final order on July 13, 2017, with respect to a workplace located at 210 E Ashland Street, Morton, IL 61550.

Noe Cruz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1269403, citation number 1, item number 1a and was affirmed as a final order on December 11, 2017, with respect to a workplace located at 7724 N Knoxville Ave, Mt. Hawley Country Club, Peoria, IL 61614.
