29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  KG Home Improvement, LLC, 11 Pershing Dr., Plainville, Connecticut; As of November 20, 2018 the employer failed to provide certification that the following citation items had been abated:  a.  Citation 1 item 2a - 29 CFR 1926.503(a)(1) - provide fall protection training to all employees potentially exposed to fall hazards. b.  Citation 1 item 2b - 29 CFR 1926.503(b)(1) - providing a written certification of the fall protection training that includes the names of employees trained, the date of the training, and the signature of the person who conducted the training or the employer. c.  Citation 1 item 3b - 29 CFR 1926.1060(a) - provide training to employees on using ladders and stairways to recognize hazards and the procedures to minimize the hazards.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder way was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).    Ladder for roof access, 1777 Main St., Agawam, Massachusetts; On or about June 21, 2018 employees were using a ladder that was not set up at the proper 4:1 angle.  Employees were exposed to the possibility of falls from kickout or failure of the side rails on the ladder due to the fact the ladder contacted the roof at approximately 11.5 feet and the base was approximately 6.7 feet out horizontally instead of 2.8 feet.
