Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about June 21, 2018, at the site located at 424 Melick Drive, Delaware, Ohio, an employee was installing shutters on a two story home from the lower level roof with a pitch of approximately 4 in 12 and approximately 10 feet in height without the use of fall protection, thereby exposing the employee to a fall hazard.
