29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.     a.  On or about June 21, 2018, at the site located at 424 Melick Drive, Delaware, Ohio, the employer failed to maintain a safety and health program to ensure hazard detection and correction.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.      a.  On or about June 21, 2018, at the site located at 424 Melick Drive, Delaware, Ohio, an employee was installing shutters on a two story home from the lower level roof with a pitch of approximately 4 in 12 and approximately 10 feet in height without the use of fall protection, thereby exposing the employee to a fall hazard.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.     a.  On or about June 21, 2018, at the site located at 424 Melick Drive, Delaware, Ohio, the employer failed to train employees that performed residential exterior work from roofs on the recognition and mitigation procedures of fall hazards.
