On or about June 21, 2018, at the construction site located at 10348 Sawmill Road, Powell, Ohio, an employee was installing framing on a one story commercial building while working from a platform elevated by a rough terrain forklift at approximately 10 feet in height with no fall protection, thereby exposing the employees to a fall hazard.
