Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about June 20, 2018 employees installing a rubber roof on a commercial building were not protected from falling to lower levels.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: On or about June 20, 2018 individuals exposed to fall hazards were not trained on how to recognize and mitigate those fall hazards. U.S. Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1230399, citation number 1, item number 1 and was affirmed as a final order on June 6, 2017, with respect to a workplace located at 3700 S. River Road West Bend, WI 53090.
