29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 3:00pm on 06/18/2018 at 1102 N 28th Street, Tampa, Florida, an employee suffered a work related injury and was hospitalized for surgery on the same day of the injury. The employer was aware of the injury and hospitalization at 7:00pm on the same day of the incident. The employer notified OSHA on 06/20/2018 at 8:35am of the in-patient hospitalization.
