29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Production Area- On June 21, 2018, the employer failed to ensure that each powered industrial truck operator was competent to operate the sitdown forklifts safely as demonstrated by the successful completion of operator training and evaluation.  No abatement certification or documentation is required for this item.    Dynaco USA, Inc. dba Entrematic was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.178(l)(1)(i), which was contained in OSHA Inspection Number 1132704, Citation 1, Item 1, and was affirmed as a final order on April 28, 2016, with respect to a workplace at 935 Campus Drive in Mundelein, Illinois.

29 CFR 1910.136(a): The employer did not require employees to use protective footwear when employees' feet are exposed to foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, that remains after the employer takes other necessary protective measures.  a) Production Area- On or about June 21, 2018, the employer did not require the use of protective footwear by employees that handled long round metal bars.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:  a) Production Area- On or about June 21, 2018, the employer failed to ensure that all powered industrial vehicles, such as the Crown and Yale Forklifts, were examined before being placed into service.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  a) Welding Area- On or about June 21, 2018, the employer failed to ensure that all electrical equipment, such as the two drop down outlets in the Welding Area, were free from recognized hazards, including arching affects, which are likely to cause death or serious physical harm.  Both of these outlets had exposed live electrical parts and showed evidence of damage.   No abatement certification or documentation is required for this item.
