29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a)  Production Floor; On or about June 20, 2018 and at times prior thereto the employer exposed his employees to potential hazardous energies in that employees were allowed to work on including but not limited to resistance welders and mechanical power presses which have multiple energy sources such as electric, air, and water without those employees having a written machine specific lock out procedure to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine.

29 CFR 1910.217(e)(1)(ii): Records of inspections and the maintenance work performed on the clutch/brake mechanism, anti-repeat feature, and single stroke mechanism of mechanical power press(es) were not maintained:   a)  Production Floor; On or about June 20, 2018 and at times prior thereto the employer exposed employees to potential crushing and amputation hazards in that employees were allowed to operate full revolution mechanical power presses including but not limited to the #10 Verson mechanical power press that the employer had not inspected, tested, and documented the condition of the clutch/brake mechanism, anti-repeat feature, and single stroke mechanism of the press on a weekly basis as required by the standard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:     a)  Compressed Gas Storage Area; On or about June 20, 2018 and at times prior thereto the employer exposed employees to fire hazards in that employees were allowed to walk and work in the same area where oxygen cylinders and fuel gas (acetylene and propane) cylinders were stored together without a barrier as required by the standard.
