29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) The employer was operating an excavator while removing soil from a trench for the installation of a PAWC water main and did not determine the exact location of an uncovered UGI 2" gas main by using by safe and acceptable means such as but not limited to a line locator, hand held excavation, probe rod, or soft dig technology, resulting in the bucket of the excavator rupturing the gas line, exposing the operator and other employees in the vicinity to a fire and explosion hazard, on or about June 22, 2018. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.
