On or about June 20, 2018: Employees working in the back of the warehouse are exposed to struck-by and trip hazards where the back exit doors exits to an active rail operated by an adjacent company.

On or about June 20, 2018:  Employees working in the battery charging area and connecting PITs to battery chargers are exposed to electrical shock hazards where there is damage to the Ferro Five 480 volt charging cable and the cable connector housing.

29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading.   On or about June 20, 2018: Two to three temp employees working in the dock area were exposed to serious injury when the trailer they were loading pulled away from the dock door.  This incident occurred on June 19, 2018.  The employer did not have procedures in place to include but not limited to the use of chock blocks.

On or about June 20, 2018: The employer did not ensure that the 300 Log for 2018 was filled out in detail as required by the standard. (a) For 5/28/18 injury -  lifting boxes hand, wrist and forearm are hurting.  No other description of the injury was given. (b) For 6/19/18 injury - slipped on a shipping paper packet from one of the boxes.  No description of the injury was given.
